If you have returned from Wuhan in the last 14 days:

and develop a fever, difficulty breathing or a cough within 14 days, you should seek medical attention. Please stay indoors and avoid contact with others where possible, ring 111 informing them of your symptoms. Please DO NOT come to the practice.

Text Messaging Reminder Service is now live!!
This means, where we have your mobile number, we will be able to set a reminder to be sent about appointments. If you wish to use this service, please let the receptionist know next time you visit the surgery and we can check we have the up to date details for you.
You can also download the MJog App onto your phone, this allows us to send appointment reminders directly to you via the app. If you want to download this, please click the button below
Advanced Nurse Practitioner
Our Advanced Nurse Practioner (Jenny) will be triaging all requests for urgent/on the day appointment requests. When you call the receptionist will ask for a brief description of the problem. This will enable us to direct you to the most appropriate care. If you do not wish to give this, we will respect your decision. Jenny will then call you back to discuss and arrange an appointment where necessary.

St Michael's Hospice Mobile Information Point
The St Michael's Hospice Mobile Information Point will be visiting Ewyas Harold on the last Wednesday of each month from 10am to 1pm. The next visit will be in January 2020. Please pop down and say hello to the staff and see what they are offering.

This is a community development project, now in year 2 of a 3 year lottery funded initiative. Opened in 2017 the hubs aims include Improving Wellbeing, Developing Peoples Skills & Confidence and Building a Strong Community. On offer is a library and a cafe amongst other things. Military Veterans Covenant
GP Practices are supporting the initiative to become 'Veteran Friendly'. As per the Armed Forces Covenant, all veterans are entitled to priority access to NHS care (including hospital, primary or community care) for conditions associated to their time within the armed forces (service related). However, this is always subject to clinical need and does not entitle you to jump the queue ahead of someone with a higher clinical need. If you have ever been a serving member of the Armed Forces do please let the Reception Team know and they will ensure your records are annotated with this information.
Your Data Matters to the NHS
Information about your health and care helps us to improve your individual care, speed up diagnosis, plan your local services and research new treatments.
In May 2018, the strict new rules about how this data can and cannot be used were strengthened. The NHS is committed to keeping patient information safe and always being clear about how it is used.
You can choose whether your confidential patient information is used for research and planning.

Herefordshire One Record
Herefordshire patients will soon benefit from an improved digital sharing system called Herefordshire One Record
This will enable sharing of patient records between GPs and other health care professionals in the county to ensure patients get the best possible treatment when needed.

Call your local GP Surgery. If we can’t see you at a time that suits you, we will refer you to a local GP or Nurse who can. Alternatively call: 0800 121 7221.
FOR MORE URGENT APPOINTMENTS CALL 111
Ask for Taurus Healthcare. They will find you an appointment quickly with a local GP or Nurse.
You will be seen at one of the following locations for routine and urgent appointments:-
South Wye Medical Centre, Belmont Road, Hereford, HR2 7JE
6.30pm – 8.30pm Monday - Friday. 8am – 8pm Saturdays, Sundays & Bank Hols

8am – 12pm Saturdays, Sundays & Bank Hols

All patients registered with the Golden Valley Practice have an allocated General Practitioner to look after their care. Patients do not have to see this GP, they are welcome to see whichever GP they prefer. If you wish to know who your allocated GP is, please telephone the Surgery.
Patient Online Access to Medical Records
Patients are now able to request access to a summary of their Medical Records. This will only include medications, allergies and adverse reactions.
If you already have an Access Pin, you will now be able to view this information.
If you do not have an Access Pin and would like access to your medical record, to book appointments and order repeat prescriptions online please speak to reception.
There are changes occurring in how we protect the confidential and personal information that we record in your medical records. The changes make it a legal obligation for us to share your information. We feel it is vital that you as our patient are made aware of these changes. This information will be stored on national secure servers and will be managed by NHS England. If you don’t wish your information to be extracted then you MUST inform your GP Practice who will then block the uploading of your identifiable and personal information to the Health & Social Care Information Centre.
